This is a good option for those of you who do not have visible veins in your legs and are uncertain if the symptoms you are experiencing are from vein disease.

A complete one-on-one evaluation with our experts (full doppler + appointment with one of our providers)

This is a good option for those of you who have had vein problems in the past or have visible veins in your legs. You are pretty sure you have a problem.

Please still submit the form below. When someone from our office calls you, please tell them you have visible veins. We will have you skip the brief screening and go right to the thorough ultrasound/appointment with one of our physician’s/PA’s.

FAQ’s on Vein Evaluation

Is my evaluation covered by Insurance?

The 10-15 minute ultrasound appointments are FREE. There is no cost associated with this brief screening. Should you schedule a more thorough evaluation (option #2), the visit portion of this is FREE, however, we will need to bill your insurance for the ultrasound portion.

Why wouldn’t I just come to the FREE 10-15 minute screening then?

This screening is a pre-screening, this ultrasound is NOT as time extensive as the one that will be done at your one on one scheduled appointment. We simply like to offer this screening for our patients as a proactive approach, for those who are uncertain if they have a vein problem. If the brief pre-screening shows you have a problem, your next step will be to come in for the thorough evaluation to talk about your treatment plan.

How do I know what will be covered by insurance for my ultrasound at my one on one appointment?

Simply call our office and ask to speak with Julie, our billing manager, and she will help get your questions answered prior to your appointment. This way you know what to expect prior to coming in.

Board Certified General Surgeon

Dr. Koziarski was born and raised in Toledo, OH. He obtained his undergraduate education at the University of Toledo, and then attended the Medical College of Ohio to obtain his MD degree. Following medical school, he completed a 5 year residency at the Medical College of Ohio in the study of General and Vascular Surgery. During his residency, he trained in one of the first freestanding, dedicated vein clinics in the country.
